Liverpool was transformed from a small town of 7,000 people in 1800 to the second city of the British Empire, with over 350,000 residents, by 1851. Despite having the country's highest death rate and being a place of violence and disease, Liverpool was home to the first integrated sewer system in the world and the first medical doctor of health in the country.
